If you wish to make an appointment at Prostate Screen Queensland, you may call Jan Higgins to arrange an appointment. Once an appointment has been made you will be sent the following via email, post or fax:

• Registration form: this contains information about your contact details, health insurance and Medicare details, together with your past medical and surgical history
• Information on Prostate Cancer Testing: This is an information sheet on prostate cancer screening (testing) and it is recommended that you read this prior to your consultation.
• Informed Consent Form: We require that you sign an informed consent form at the end of the consultation with our doctors and prior to undergoing prostate cancer testing. This indicates that you have read the information sheet on Prostate Cancer Screening, had a discussion with the doctor and had all of your questions answered. This will be sent to you at the time of your booking but is signed after your consultation with the doctor.

All 3 documents of these documents should be brought with you for your consultation so that any specific questions you have can be answered.
